$40 yearly membership fee per person (due upon enrollment-cash or money orders only)
No monthly premium
Affordable co-payments payable at time of service

How can I apply?

Apply at a Community Health Center of your choice or call PCAP for additional options.

What documentation do I need to participate in PCAP?

Proof of income: employment, pension, unemployment (current & last months)
Proof of residency in Pima County

When am I eligible?

Qualified members will be effective immediately upon enrollment.
